Important Notice: 1095-C Tax Form Availability 

IRS Form 1095-C reports the health coverage offered to you during the calendar year.  Based on recent IRS guidance under the Paperwork Burden Reduction Act (PBRA), Richmond Public Schools will no longer automatically mail Form 1095-C to all full-time employees.  Forms will be available electronically and upon request. 

What you need to know:

ACTIVE EMPLOYEES

Access your Form 1095-C online through the benefits portal (available 24/7). Click here to access the benefits portal. Your pin is a six digit number, consisting of the last four of your SSN, followed by the last two digits of your birth year.

 INACTIVE EMPLOYEES

Request a mailed copy of your Form 1095-C using one of the options below:

o   Phone: 844-379-0069

 

Processing timeline: Forms will be provided within 30 days of your request or by January 31, 2026 - whichever is later.
